      • 1. The floor in the kindergarten to Grade 4 classroom was in disrepair.* Repair the floor, as it can create a tripping hazard. Also, all surfaces in the facility must be smooth, non-absorbent, and easily cleanable.2. The window in the room for children ages 4 to 5 years old had a broken insect net.*Please replace or repair it. Staff have secured the window, but ensure it remains securely shut until the insect net is replaced, as the window is at ground level and provides direct access to the playground, where children could potentially access it.

      • Tape was used to secure the open edge of the blue diaper change mat located in classroom 2. Dirt/debris was observed under the diaper change pad. Please remove the tape from the diaper change mat. Fix/replace with an alternative that is smooth, impervious to moisture and easy to clean. Clean and sanitize the diaper change area.
