- Unsuitable critical equipment such as needles without lot number and expiry date were observed onsite.-Operator was advised not to use such critical equipment as the required sterility assurance documentation would be impossible to document and maintain.The indicated needles were discarded during the inspection.
- 20. Is reusable equipment disinfected properly?
- High-level disinfectant, preempt CS20, available onsite had expired since July 2024.-Please provide new and effective high-level disinfectant.If high-level disinfectant is to be reuse for more than one day, please procure suitable test strip to monitor its concentration and document the test results or discard spent solution at the end of each day.

Part of the facility's treatment floor cover are carpet (rug).-Please place a smooth, non-absorbent and easy to clean cover over the indicated area/surface.
