      • Toy play food items (muffins) were in disrepair and not easily cleanable. Discarded during inspection.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • Basement OOSC room: Paper towel dispenser does not adequately hold paper towels. Paper towels were stored on shelf next to toilet. Provide an adequate paper towel dispenser to protect the paper towel from contamination.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • There were push pins used on the walls of the classrooms. The inspector informed the staff/operator that push pins cannot be used as they are prone to falling and be accessible to children. The push pins were removed during the inspection.
    • 06. Are proper protocols being followed in regards to client care?
      • There were no chlorine test strips available on site. The inspector informed the operator that test strips must be used to confirm chlorine concentration.**OBTAIN CHLORINE TEST STRIPS.

      • The chlorine sanitizer solution was measured to have 1000-ppm chlorine, and the staff was not rinsing the surfaces afterwards. The inspector informed the staff/operator to either keep the chlorine sanitizer solution between 100-200 ppm or add a rinsing step afterwards when using concentrations above 200-ppm.

      • Some paint and glue bottles did not have an AP (Approved Product) symbol. The inspector informed the staff/operator that all paint and glue products must have the AP symbol to ensure that these products were tested for non-toxicity to children. The products were discarded during the inspection.

    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1. Torn diapering mat was observed in the baby and toddler's room.-Please replace the torn diapering to ensure smooth surface necessary for proper cleaning and disinfection of the mat.2. Dirt was also observed under the diapering mat.-Clean and sanitize under the diaper mat and its surrounding area after every diapering process.
    • 26. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Critical)?
      • Mouse droppings were observed in a cluttered area adjacent to the facility's kitchen.-Please consult with pest control expert to find and block all entry of pest (such as holes, gapes or crevices) in the daycare facility.-Clean and disinfect all surfaces and materials including children's sleeping mats, stored in the indicated area with freshly prepared 1000 ppm of bleach solution.-Please declutter this indicated area to ensure all surfaces can be seen, clean and sanitize at all times.

      • Dirty toy containers were observed in the preschool room.-Please clean and sanitize all toy containers and ensure they are maintained in sanitary condition at all times.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • Heat (air) register in the preschool room was blocked with dirt from art and craft play materials.-Please clean the indicated heat/air register to prevent smoke or fire hazard. Also, prevent future blockage of this register to ensure proper air and heat circulation in the room.
