    • 20. Is reusable equipment disinfected properly?
      • Records of high level disinfectant (HLD) testing were not being maintained. Test strips were available and operator said that they were testing the HLD every day, but not logging the results. Please ensure results of HLD testing are recorded and kept for verification purposes.
    • 20. Is reusable equipment disinfected properly?
      • The high level disinfectant (Pre-Empt CS20) being used had expired in 2022. A new bottle was available on site for use. The expired bottle was thrown away during the inspection. No equipment was being reprocessed at the time.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • An unlabelled bottle of clear liquid was seen during the inspection. Operator informed PHI it was alcohol. PHI asked operator to label bottle with a sharpie, which operator did during inspection. Please ensure all bottles are labelled with their contents.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The massage table cover was worn in places and was no longer smooth, non-porous and easily cleanable. Please repair or replace cover.

    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Some nail files were being disinfected between use. Porous items cannot be disinfected as they absorb chemicals, as well there is an inability to remove all physical contamination. The nail files were discarded.
    • 20. Is reusable equipment disinfected properly?
      • Pre-empt CS20 was expired. Pre-empt RTU was being used to disinfect semi-critical items. Pre-empt RTU is only for non-critical instruments as well as surfaces. If Pre-empt CS20 is to be used, it is to be tested and logged each day over a maximum 14-day re-use period. Otherwise, it can be poured and discarded after daily use.
