Client records for personal service that use critical equipment were not maintained or kept accessible for up to two years.-Please develop consent form and keep client records for the required period.2. Sterility assurance documentation such as needle's lot number, expiry date and manufacturer's information were not maintained with client's record.-Please retain the indicated documentation with client's record for at least two years.
