    • 11. Is post-service care provided by personal service workers following any personal service or accident where a client's skin or mucous membrane is cut or punctured during a personal service, or when energy-emitting equipment is used?
      • There was no written after-care treatment instructions/ post-care procedures provided to clients after tattoo service.-Please develop post-care instructions and ensure the instructions are communicated and accessible to clients.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • 1. Client records for personal service that use critical equipment were not maintained or kept accessible for up to two years.-Please develop consent form and keep client records for the required period.2. Sterility assurance documentation such as needle's lot number, expiry date and manufacturer's information were not maintained with client's record.-Please retain the indicated documentation with client's record for at least two years.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Alcohol used to clean and disinfect surfaces that could be contaminated with blood.-Please procure and use low-level disinfectant such as cavicide or cavi wipes to ensure proper cleaning of surfaces and tattoo machine.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The window of the treatment room had no screen to prevent entry of insects.-Please provide suitable window screen to ensure adequate ventilation in the treatment room and prevent pest entry.
