      • 1) OSC - chemicals were observed to be stored in the cabinet beneath the bathroom sinks. Paint, toilet cleaner, carpet cleaner and other chemicals were being stored in an area accessible to children. Please ensure all chemicals are stored properly out of the reach of the children.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1) Preschool room - the white shelf was not secured from tipping. Please ensure the white shelf is secured from tipping.2) Preschool room - the blue shelving unit was not secure from tipping. Please ensure all shelving over 4 ft is secure from tipping.
    • 06. Are proper protocols being followed in regards to client care?
      • 1) Infant room - staff indicated that diapering was being conducted in the middle of the room, near the eating areas and not by the designated hand sink. 2) Infant room - Diaper pad was leaning against the crib. Please ensure that the diaper pad is stored in a designated location away from food and sleeping.3) Infant room - There was no bleach solution of the diapering area.

      • 1) Toddler bathroom - there was a missing baseboard in the bathroom.2) Toddler bathroom - there was a water stained ceiling tile in the bathroom. Please ensure leaks are not present and replace the ceiling tile.3) Toddler room - there was a baseboard coming away from the wall. Repair required.4) OSC - the wall behind the door was in disrepair 5) OSC - the wall was in disrepair behind the door in the boys washroom. Repair required.

      • 1) Toddler room - the change pad was dirty and peeling. Replacement required.

      • 1) Toddler room - the white shelving in the toddler room was in disrepair.2) Toddler room - the gate was broken at the entrance to the toddler room. Repair required.3) The gate to separate the toddler and preschool rooms was broken. Repair or replacement required.4) OSC - light switch covers in the bathroom were visibly dirty. Cleaning required.5) OSC - The bathroom vanity in the first washroom had exposed wood and was in disrepair. Repair required.

      • 1) Infant room - pushpins were observed in the room. Please refrain from use as pushpins could be potential choking hazards if they fall out of the wall.2) Infant room - an electronic stuffy dog was observed in the room. Please remove as these items cannot be properly washed.3) Infant room - Dirty and broken toys were observed in the room (baby toys, football, stuffies torn with fluff inside)4) Infant room - staff indicated that toy washing was being conducted in the diapering hand sink in the infant room which could increase the risk of cross contamination and does not allow for proper cleaning.5) Infant room - Dirty mouthed toy bin was not being used.6) Infant room - Toy cleaning checklist was not present and no sanitation plan was available.7) Toddler room - pushpins were observed in the room. Please refrain from use as pushpins could be potential choking hazards if they fall out of the wall.8) Toddler room - Some toys were dirty, broken or in disrepair. Playskool bead toy visibly dirty. Puzzle boards were worn and in disrepair. Please discard. The pink bench was dirty inside and requires cleaning.

      • Multiple unlocked cabinets beneath handwashing sinks contained chemicals such as bleach or toilet cleaner. A few hours after the inspection, Maninder submitted photos showing that all the cabinets have had child-proof locks installed. Please ensure all hazards are inaccessible to children.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • The large play-kitchen set in the preschool room had some visible grime buildup on high-touch areas. A few hours after the inspection, Maninder submitted photos showing that the playset had been cleaned. Please ensure the larger toys and playsets are also cleaned regularly.
