    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility specific written procedures were not completed. AHS template for applicable facility specific written procedures were discussed and left onsite to be completed.client care, skin cleansing, post care and antiseptic application. Post care procedures were available where applicable.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Client records maintenance was discussed onsite Client consent forms were available to record client details. Sterility assurance documentation for presterilized single use cartridges, sharps needles were reviewed onsite.Ensure sterility assurance documentation including batch number, date of expiry lot number are recorded for all presterilized single use items and retained with client records.
    • 20. Is reusable equipment disinfected properly?
      • High level disinfectant CS20, and intermediate level disinfectant wipes PreEmpt passed their expiry date. Obtain a new CS20 with valid expiry date. Ensure all disinfectant and supplies have valid expiry date.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • Manuals for several equipment were not available during inspection.Ensure manuals are available for all equipment and all equipments are maintained according to manufacturer's instruction for use and maintenance.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Beyond use dates were not marked on the cosmetic products. Some products expired already were discarded during inspection onsite..Ensure all products with beyond use date are marked with their expiry date after first opening.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Reprocessing sink was installed in the kitchen area. which will be moved in the hallway outside the bathroom and water supply and drain will be connected to bathroom hand sink.
