    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • The operator lacks knowledge in the following areas:1. Legislative requirement for disinfection, general cleaning and piercing.2. Safe delivery and sterility requirements for piercing.3. Record keeping and aftercare requirements.- Read personal services regulation and standards. - Create a cleaning schedule for general areas. - Create cleaning protocols for after service is complete.- Create a piercing protocol listing all steps in the piercing process and include skin antiseptic, sterility requirements, safe handling of sterile equipment, client consent, sterility tracking, and aftercare.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • There was no paper towel available for hand washing. Install paper towels in a dispenser or single use reusable towels can be used.
    • 20. Is reusable equipment disinfected properly?
      • 1. The operator was disinfecting non critical surfaces with alcohol. 2. The operator was disinfecting non critical equipment, like the tattoo machine, with alcohol.3. The operator was disinfecting intermediate tools, such as lash tweezers and facial tools, with alcohol.4. The CS20 onsite expired 2020. -Purchase low level disinfectant for surfaces and non critical tools. - Purchase high level disinfectant for intermediate tools.- Discard expired CS20.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • 1. Jewelry used for piercing was disinfected with CS20 and packaged. Jewelry must be sterile at the point of use either in house using a sterilizer approved by the PHI or purchase sterilized.2. Sterility information for needles and jewelry was not tracked on all client records. Needle and jewelry lots must be tracked with client records.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Lidocaine with no DIN or expiry onsite. When purchasing lidocaine ensure it has a DIN and expiry.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1. There was a build up of ink on the tattoo equipment storage cart and supplies within it. 2. There was a build up of wax on the wax pot.3. There was a build up of debris on the client chair, in the facial water reservoir and on the nail curing light. Ensure these areas are cleaned. If they are not cleanable remove items. Discard any porous materials that have been contaminated with ink such as grip tape.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • 1. I.V catheter needles used for piercing. Remove I.V. catheter needles from site. If continuing piercing purchase needles and tubes that indicate on the manufacturer's instructions for piercing use and are sterile at the point of use.2. After care instructions for piercing recommend the use of tea tree oil to clean the piercing. Redevelop aftercare instructions to include a cleansing step for piercing. Refer to my health website for piercing care.3. Remove any services and equipment not in use.

    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The antiseptic wipes used are expired. Please replace.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Items labelled as single use such as single use skin markers were reused. Please ensure single use items are discarded immediately after use.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • The manufacturer's instruction for use for the high frequency machine was not available onsite. Please ensure manufacturer's instruction for use for the high frequency machine is available onsite.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • The manufacturer's instruction for use for microneedling machine was not available onsite. Please ensure the manufacturer's instruction for use is available onsite.
