      • Facility specific written procedures were not in place. Complete the following written procedures and ensure they are easily accessible for all staff. 1. The cleansing of skin and mucous membrane. 2. Client care in the event of an accidental cut or puncture.3. Decontamination of any sink that will be used for both handwashing and equipment reprocessing

      • One sharps container overfilled. Do not fill pass fill line.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Biorepeel onsite that contains TCA. TCA products are banned. Remove product.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Botox fridge measured -10 C. Thermometer for fridge was dead. Replace batteries. Discard product if fridge reached below MIFU low level temp.

      • Facility specific written procedures were not in place. Complete the following written procedures and ensure they are easily accessible for all staff. 1.     • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • The sterility information (lot number and the expiry date) of the pre-purchased sterile equipment (needles, cartridges) is not being kept with that client's records. Please ensure the sterility information of the pre-purchased sterile equipment used on the client is documented with the client's forms.
    • 20. Is reusable equipment disinfected properly?
      • Disinfectant without a DIN (IPA Sanitizer wipe) is used to disinfect noncritical tools and surfaces. Please ensure at least a low-level disinfectant is used to disinfect non-critical tools and surfaces.
