    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures for the following tasks:-the cleansing of skin and mucous membrane -client care in the event of an accidental skin cut or puncture-decontamination of any sink that will be used for both handwashing and equipment reprocessingProvide written procedures for the following issues.
    • 20. Is reusable equipment disinfected properly?
      • Indicator strip fails to produce the required measurement; no disinfectant was detected in the PREempt CS 20 tools soaking tray. Disinfectant was used since July 16, 2025. No test strips were available to test the solution daily. Discard PREempt CS20 at the end of the day or provide test strips and test the PREempt CS20 to make it produce the required measurement. Log the measurement daily. Previous Inspection:1) Manicure and pedicure tools were immersed in 70% alcohol. 2) PREempt CS20 disinfectant expired October 2021 and was covered dust. 3) Lemon Gard liquid cleanser expired June 25, 2022. No Test strips were available to confirm if the right concentration is used. 4) Nail drill bits were not cleaned or disinfected after use. Found dirty drill bits at the manicure table drawers. Provide high-level disinfectant and follow the manufacturers' instructions use.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • 1) Dryer ducts were broken, dust build-up in the dryer area.2) Manicure table drawer covers were peeling off. 3) One of pedicure chairs was covered with tape. 1 to 3) Address these issues.

    • 16. Is reusable equipment handled and stored properly (Non-Critical)?
      • Bed sheet was used to cover the waxing table, and it's not laundered after each client.Launder sheet after each client.
    • 18. Is reusable equipment cleaned properly (Non-Critical)?
      • 1) Used files and callus scrubber were found in the manicure drawers.2) Used disposable Pedicure Slippers were found in the in the pedicure carts. 3) Used sterilization bags were found at the processing counter. 1 to 3) Do not reuse items that are intend for one time use.4) Brushes and manicures tools at the manicure stations were covered in nail dust.Store these items in a location to prevent them from getting dirty.
