Bingo dabbers without non-toxic labeling were found in the classroom. Staff removed the bingo dabbers. Please ensure the art supplies have a non-toxic claim. Violation corrected onsite.3. Rooms 202, 204: Electrical cords were accessible to the children. Ensure all electrical cords are kept out of children's reach. Violation corrected onsite.

      • Room 201: The CPR valve is missing from the first aid kit. Please ensure first aid kits are equipped with necessary supplies.

      • 1. Room 201: Extra beddings were overflowing from the bin and stored by the children’s cots. Please ensure extra beddings are stored in a sanitary manner.2. Rooms 201: The diapering mat is stored by the toilet. Please ensure a dedicated space is made to store this mat sanitarily.

      • 1. Rooms 101, 103, 205: Used cleaning rags were hung on the faucet or left out on the counter. Please ensure the following:- cleaning rags are used only once and then disposed of or laundered. This prevents cross-contamination and bacterial spread. - place clearly labeled bins in each classroom for used rags.2. Rooms 201, 204: The diapering mat is torn exposing the inner material. Please replace.

      • 1. Classroom 204: Cleaning rags were hung on the faucet. Please ensure the following:- cleaning rags are used only once and then disposed of or laundered. This prevents cross-contamination and bacterial spread. - place clearly labeled bins in each classroom for used rags.2. Bleach solution 100 ppm was used to sanitize surfaces. During an outbreak, please switch to 1000 ppm bleach solution. 3. toddler classroom: Plush toys were accessible to children. Please ensure anything with fabric is taken out of the classrooms during an outbreak.

      • There were gaps under external doors (entrance area and the loading dock area).-all exterior doors must be tight fitting and/or install door sweeps/weatherstripping.-UPDATE: will be resolved during the Dec closure

      • There was no liner used for a diaper change; staff are not following diapering procedures. -

      • 1) The securing wall attachment for the blind cord in room 202 was missing-install the wall attachment for the blind cord.2) Walls throughout daycare were damaged (ongoing maintenance)-ensure all walls are repaired and the surface is smooth, durable, washable, etc. For walls that are constantly damaged, consider installing FRP covering. 3) Mats in Room 204 were deteriorated. -replace any damaged equipment.

      • There were gaps under external doors (entrance area and the loading dock area).-all exterior doors must be tight fitting and/or install door sweeps/weatherstripping.

      • Dishes and cups were being rinsed/washed in the handwash sinks in the class.-ensure all dishes, cups, cutlery, etc are washed & sanitized in the kitchen using the dishwasher.

      • There are mirrors on the north wall of gym room 104 (was previously used as an office). Unknown shatter proofing measures, mirrors are within 1m of the floor.- Shatter proofing measures are required on any mirrors within 1m of the floor in childcare areas. Provide documentation showing mirrors are shatterproof or install shatterproof film.

      • Bleach spray bottles are not consistently maintained properly - were empty in one room, mislabeled/not labeled in other rooms, were not properly designated for incompatible uses.- Ensure each room has a disinfectant solution available. Bottles must be properly labeled.Ensure there is one bottle designated for diapering/toileting and a separate one for tables/toys.When cleaning toys in classroom sinks, soap and water is mixed in the same sink with bleach.- Separate steps are required for cleaning and disinfection. Steps should include clean with soap and water, rinse, disinfect with 100ppm bleach solution, air dry.

      • Items such as bibs, aprons, educational materials were stored in toilet stalls.- Do not use toilet stalls for storing anything other than toileting or diapering supplies.

      • Evidence of mice in the facility - room 101 and kitchen. - Please clean these areas thoroughly and continue to monitor. If current methods do not eliminate the problem, follow up with a pest control company may be required.
