      • The portable oil deep fryer was used without a proper canopy hood. Remove the oil deep fryer or switch it to a self-contained deep fryer that does not require a canopy.Cardboards were used to line to back prep table. The cardboards were discarded on site.Use something that is easily cleanable, smooth, and water resistant.

      • Scented bleach was used to make food contact surfaces sanitizer. Plain bleach was also available on site.Ensure to use only plain bleach and water to make 100ppm sanitizer solution for dishwashing and sanitizing food contact surfaces.

      • The dough mixer turning rod and the bowl were observed with food debris.Clean and sanitize.The chest freezer in the middle of the kitchen was observed with heavy ice build up.Defrost and clean.

      • Many personal use items were stored in the kitchen and the back storage space. Move the personal use items to the staff room or remove from the premise to reduce clutters.

      • Operating without a valid food permit. Please obtain a food permit.

      • Grease and stains build-up on cleaning cloths. Discarded the cleaning cloths. Replace cleaning cloths when they can't be cleaned.
    • 08. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Non-Critical)?
      • Tools and personal items were stored along the pizza boxes storage shelves. Provide a separate space for tools and personal items.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • No hot water at the handwash sink, hot water tap was shut off, Dishwashing sinks are available for handwashing.Fix the hot water tap and ensure there is hot and cold water for handwashing.

      • REPEAT VIOLATIONS:November 14, 2023, inspection: A mouse was found in one of the mouse traps. Remove mouse and increase pest controlPrevious Report1) Few mouse droppings were found in the back storage area shelves. Clean the mouse droppings and increase pest control.2) Personal clutter was stored in the front area.Remove all personal clutter.

      • REPEAT VIOLATION:Again, used paper towels were scattered throughout the kitchen.Discarded all the used paper towels.Please place the paper towels in the garbage after use.Previous Report:Used paper towels were left on the prep counters. Discard paper towels in the garbage bin after use.

      • Clear hose attached to the sink tap. Remove the hose.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • 1) Broken Stand up freezer was stored in the kitchen and duct tape was used to repaired with the gasket.Fix the stand up freezer or remove from premise. 2) Plastic corrugated sheets were not able to be cleaned and sanitized. The black debris build-up along the edge openings. These sheets were used to cover pizza dough.Do not use plastic corrugated sheets to cover pizza dough. 3) Used small appliances were stored in the office space. Owner is not sure if they work or not. Discard all broken appliances.

      • REPEAT VIOLATION:November 14, 2023, inspection:More personal clutter accumulated throughout the premise. Previous inspection:Personal clutter stored in the facility. Remove all personal clutter.

      • 1) Cardboard boxes and plastic bags were scattered throughout the facility. 2) Cardboard boxes were used to line the pizza prep table. 3) Dust on the ledge above the oven and stand-up fan on the ledge. Dead flies on the fly strip above the oven. 1 to 3) Declutter and clean the kitchen.
