    • 06. Are proper protocols being followed in regards to client care?
      • 1. In Room 1 the diaper mat has cloth on the side. This material is not cleanable. **Diaper mat was replaced with a plastic wrapped diaper mat. Monitor the mat and replace if the plastic cover is ripped or worn.**2. In Room 3 the buckle for the diaper change table was broken.**Replace with a new buckle.**3. It was found that children hands were not being washed after a diaper change. The belt buckle was not being wiped down.**Ensure childrens hands are washed after a diaper change. Ensure the whole mat including the belt buckle are also disinfected.**
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1. Room 1 had a case of confirmed HFM yesterday. Floor mat was not washed. **The floor mat was put in the laundry right away.****Ensure after a confirmed case or suspected case all toys, surfaces, and mats are cleaned and disinfected.**2. The fake turf in the indoor playground is hard to disinfect daily. The staff uses a carpet cleaner to clean the grass once a week. Spoke with Owner and is planning to change out the turf. **Suggestion made to the Owner that if a class has a suspected case or confirmed case of HFM/GI that class should not be using the indoor playground.**
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • The cubbies at this facility are starting to get packed and children's jackets and other personal items are starting to touch. **Owner has bought tote bags to store all of the children's things then will hang in the cubby. Owner has also ordered new cubbies with dividers between each client.**
