- 1. Knives stored on magnetic strip had debris on them.
- 2. Biofilm formation inside the ice machine.
- Corrective Actions:
- 1. Debris on knives can contaminate food and cause illness. Knives were cleaned and sanitized by the operator.
- 2. Biofilm can contain pathogens which can contaminate ice and cause illness. Ice machine was cleaned and sanitized by operator and ice discarded.

- 1. Handwashing sink close to the ice machine had utensils stored on it.
- 2. Three handwashing sinks in the kitchen had no paper towels.
- Corrective Actions:
- 1. Handwashing sinks need to be available and accessible at all times to encourage good hand and personal hygiene to prevent transmission of diseases. All stored utensils were removed away from the sink.
- 2. Appropriate hand washing and drying prevents transmission of diseases. All three handwashing sinks were immediately supplied with paper towels.

- 1. Food items in food preparation coolers were not covered.
- 2. Food in inserts were stocked in food preparation cooler.

- 1. food needs to be covered to prevent contamination. Food items were immediately covered by the operator.
- 2. The bottom of insert can contaminate food and cause illness. The insert on top of the stack was immediately removed by the operator.

- Observation (CORRECTED DURING INSPECTION): Food debris observed on one knife stored on the magnetic strip and on the bow of the dough mixer.
- Corrective Action(s): Clean and sanitize the aforementioned knife and dough mixer. Ensure food equipment is cleaned and sanitized properly after use.

- Observation (CORRECTED DURING INSPECTION): 1. Quats sanitizer spray bottle was measured with 0 ppm Quats. The test stripe showed 200 ppm then turned to 0 ppm after 3 seconds. Operator stated there might be a mix of Quats solution with vinegar and bleach solution in the bottle.
- 2. Low temperature dishwasher dispensed 0 ppm bleach sanitizer at the plate level.
- 3. Glass washer dispensed 0 ppm iodine solution at plate level.
- Corrective Action(s): 1. Operator discarded and refilled the sanitizing spray bottle with 200 ppm Quats sanitizer. Do not mix chemical solutions together as it may decrease the efficacy of the food contact surfaces sanitizer.
- 2. Operator switch out the chlorine sanitizer stock bottle and primed the line for low temperature dishwasher. 100 ppm bleach sanitizer was measured at the plate level. Clean and sanitize all plates and utensils on site.
- 3. Clean and sanitize all glasses in the dishwasher. Do not use the glass washer until serviced and iodine concentration verified.

- 2. The hot water tap for the hand washing station at the end of the cook line was broken and unable to be turned.

- 2. Service / fix the hot water tap to ensure hot running water is provided at aforementioned hand washing station.
