      • Ensure all electrical outlets are in good repair. One corner noted to be missing for the electrical cover. Ensure all plus are in good repair.

      • Wall in the bathroom in disrepair. Exposed drywall noted. Please ensure all walls, floors and ceiling is maintained in good repair.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kit noted to be missing alcohol swabs. Ensure first aid kits are complete.
    • 15. Is laundry being properly handled in a suitable manner?
      • Laundry room needs organizing and cleaning. Please relocate refrigerator from laundry room to the kitchen. It was noted that the refrigerator in the laundry room is used for storing food needed for the kitchen.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Overall sanitation observed to be needing improvement. Please ensure organization of the drawers is completed as well.

      • Faucet in disrepair. Please fix and ensure all equipment is maintained in good repair.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • Air vent in the washroom noted to be dusty and with spider webs. Ensure all areas are cleaned on a regular basis. No cleaning records observed.

      • Ensure all cots are labeled. Several cots noted to not be labeled. Ensure personal items are kept in a safe matter, protected from contamination. Ensure stacked cots are not used for storing personal staff items or sensory items for play.Diapering table noted to be in disrepair. Ensure diapering table is in good repair.Diapering mattress noted to be stained. Diapering mattress noted to be stored on the floor in the bathroom withing children reach. Ensure diapering mattress is in good repair, clean and out of children reach.

      • Infant room - the diapering change pad was being stored on top of the cribs in the infant room. A high chair was also stored on top of the change pad. Please ensure that diapering remains separate from sleep equipment, and food and food related materials. A dedicated storage location should be available for the diaper change pad and a dedicated area for diapering if changing diapers on the change pad on the floor.

      • Toddler Room - Walls with chipped and in disrepair near the bathroom area. Repair and finishing required.Toddler Bathroom - The wall behind the sink and side of the cupboard in the toddler bathroom was in disrepair. Repair and finishing required.

      • OSC Room - Several tiles around the sink in the OSC were missing and the surface was not smooth and cleanable. Pleas ensure that the surface is smooth and cleanable.OSC Room - areas on the surface of the computer table had exposed wood. Please refinish these areas so that the surface is smooth and cleanable.

      • - Used cleaning cloths observed stored on top bleach spray bottlesInfant room - the green chair and animal print chair covers were visibly dirty. Please ensure these chair covers are laundered weekly.

      • Staff was observed changing diapers and wearing gloves but did not change gloves or wash hands in between changing clients diaper changing. Pleas ensure that hand washing is conducted in between clients even when wearing gloves.

      • OSC Room - The yellow shelving unit near the loft area is not secured from tipping. The shelf may be secured to the wall or placed horizontally to prevent a tip hazard.
