      • **** Outstanding violation ****Facility procedures describing how skin is prepared and cared for in the event of an injury have not been completed. Vietnamese and English versions of the AHS template have been provided again via email. Complete and send for review within 1 week of receiving the report.

      • Used nail files, sanding bands were stored in the manicure table drawers and storage. Ensure single-use items such as nail files, sanding bands that cannot be cleaned and disinfected properly are discarded after use.

      • **** Recurrent Violation ****Tools that have already been disinfected in the HLD5 located in the reprocessing room were visibly dirty/had organics caked onto the surface. If tools are not adequately cleaned, then they cannot be thoroughly disinfected.

      • **** Recurrent violation ****1) The tools are not dried prior to placing tools in the high-level disinfectant solution. Ensure all tools are washed, rinse, and dried prior to immersing them in HLD5 disinfectant solution.2) Operator indicated that there is no additional rinse step after tools are immersed in HLD5, which is indicated on the disinfectant's manufacturer's instructions for use.

      • 1) The concentration of HLD5 in disinfection container was measured at 0%. The empty container of HLD5 was expired as of March 19, 2025. No other high-level disinfectant available at the time of inspection. 2) The HLD5 concentration is not being tested daily, no test strips or records were available. - The solution must be discarded daily or discarded after 14 days or sooner as dictated by Test Strips.

      • 1. Tools that have already been disinfected in the HLD5 located in the reprocessing room were visibly dirty/had organics caked onto the surface. If tools are not adequately cleaned, then they cannot be thoroughly disinfected.2. Pedicure fans had debris residue on surfaces, indicating that they are not physically cleaned to remove any buildup.

      • 1. Operator was unsure if the HLD was replaced daily. Inspector's test strip was slow to change, indicating that the solution may have been over a day old. There were no test strips or record sheets onsite for measuring concentration every day that the solution is used. 2. The Preempt RTU solution is wiped immediately after application. Operators are not allowing surfaces to remain wet with the disinfectant for the prescribed 3 minute contact time required to achieve disinfection. 3. Operator indicated that there is no additional rinse step after tools are immersed in HLD5, which is indicated on the disinfectant's manufacturer's instructions for use.

      • Soaking container for HLD5 is not long or deep enough to fully immerse the pedicure foot files. Please acquire a larger container for the foot files. Test strips onsite were intended for CS20, not HLD5. Please acquire suitable test strips in order to reuse the HLD5 solution up to 14 days.
