      • Observed opened and refilled lip filler syringe packages in cupboards that were being stored and saved for same patients. Requested that these be discarded and practice ceased.

      • One vial of opened Xlylocaine that indicate a shelf life of 3 days once opened, with no label when they were opened. When asked, informed it was opened more than 3 days ago. Requested product be discarded and ensure manufacturer's instructions are followed.
