The operator must ensure that facility-specific written procedures are established that describe steps for all processes completed in the facility. Please create and implement written procedures in accordance to the Personal Service Standards.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Impellers at both pedicure foot basins were visibly dirty. The cover was removed from one and revealed a buildup of lint, and debris. The magnetically attached impeller was removed, however the cover was stuck due to a buildup of debris and lint.Please ensure impellers, are disassembled, cleaned, then disinfected between every client.Impellers making contact with pedicure water must be cleaned and disinfected even with the use of single use disposable liners.

      • - Cleansing of the skin is not being done prior to waxing and threading.Ensure cleansing is done.The operator was advised to acquire a cleanser.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Nail filers were being re-used.Single-use sanding discs were not disposed after use.- Single-use equipment was not being discarded after each use. - Porous or uncleanable scrubs were not being discarded after each use.The operator was asked to discard used single-use tools immediately.

      • **Disposable foot files were being used on multiple clients. All disposable/ single-use/ uncleanable equipment must only be used on one client and cannot be re-used on multiple clients. please ensure these items are disposed of after each client.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • - Disinfected semi-critical tools were being stored in an unsanitary box, which had nail debris and a lot of dirt, and were not being protected from contamination during handling and storage. Ensure equipment is stored in a manner to keep it clean and free from contamination.-Contaminated hairbrushes were not adequately separated and identifiable from clean brushes. Hairbrushes full of hair were kept with clean brushes in the client service area. Ensure contaminated equipment is stored completely separate from clean equipment to prevent contamination of clean equipment.

      • Previous Inspection: ** Using rubbing alcohol as a disinfectant Equipment must be classified as:- Non-Critical – Equipment that touches only hair or intact skin.- Semi-Critical – Equipment that comes into contact with mucousmembrane or non-intact skin, but is not intended to puncture either ofthem.- Critical – Equipment that punctures skin or enters sterile tissues of thebodyThis classification related to what kind of disinfection must occurs: - Equipment classified as non-critical must be cleaned and disinfected using, at aminimum, a low-level disinfectant.- Equipment classified as semi-critical must be cleaned and disinfected using, at a minimum, a high-level disinfectant.For example clippers are considered a semi-critical piece of equipment; thus they must be cleaned and disinfected using, at a minimum, a high-level disinfectant.Please ensure a high-level disinfected is purchased and used in accordance to manufactures instructions.

      • Jan 29, 2025: -Pre-empt HLD5 was expired.-Test strips were not available.-High-level disinfectant log was not maintained.-Lysol bathroom bleach was being used to disinfect environmental surfaces including pedicure tubs.-The operator informed that she changes the barbicide solution every 10-15 days.-Clippercide was not available.-The operator was advised to acquire a new high-level disinfectant along with relevant test strips.-Use of an intermediate-level disinfectant was recommended to the operator especially for the pedicure tubs.-The operator was advised to change barbicide solution every day or more frequently if required.-The operator was also advised to acquire clippercide.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • -The bleach containers and facial creams in the massage room and in the shelf next to the pedicure chair were all expired.The operator was advised to discard all the expired products.

      • There were 4 unlabeled bottles one in the hair service area and 3 in the massage room filled with unknown liquid solutions.The operator was advised to label the bottles to identify the ingredients in the bottles.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • -The sink used for reprocessing semi-critical tools was discolored and did not appear to be easily cleanable. The right side of the sink is ripping apart and have accumulation of dirt and debris.The reprocessing sink did not have enough space to promote one-way flow of the tools to be reprocessed and keep disinfected tools for air drying.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • -The facial steam machine in the spa room was observed to be unsanitary.The water in the container of the steam machine was observed to be dirty. The operator was not following manufacturer's instructions for cleaning the steam machine. The operator was using Lysol bathroom bleach to clean the water container contrary to the manufacturer's instructions.-An unsanitary single use micrfiber cloth was being used to clean the surfaces. The cloth was kept on a shelf after use and there was a foul smell coming out of it.
