- The operator providing personal services without notifying the Regional Health Authority.-Please fill and complete the Personal Services Notification form, the form is available on AHS PSN website.

- Written procedures for skin cleansing, antiseptic application and in an event of accidental cut or puncture were not available during the inspection.-Please develop and have available onsite, written procedures for:1. Skin cleansing;2. Antiseptic application; 3. Procedure to follow in an event of accidental cut or puncture; and 4. - 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
- 1. Hand sanitizer available onsite during the inspection had no DIN or NPN.-Please provide and use appropriate alcohol-based hand sanitizer.2. There were no paper towels available at the hand washing/reprocessing sink.-Please provide paper towels at the indicated area to ensure proper handwashing process.
- 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
- The antiseptic product, 99% alcohol, used for antiseptic application on the skin as well as machine handles disinfectant was without Health Canada's Drug Identification Number (DIN) or Natural Product Number (NPN).-Please provide antiseptic product with the indicated DIN or NPN for skin antiseptic application.-For machine handles disinfection, obtain 70% alcohol with a DIN or use a disinfectant with a DIN and consult the manufacturer.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- There was no sterility assurance records such as lot number, expiry date and manufacturer's name of prepackaged sterile critical equipment (needles or cartridges) for clients who undergone invasive procedures.-Please maintain sterility assurance documentation for all clients who use prepackaged sterile critical equipment. Ensure the information are kept accessible with clients' contact information for at least two years.

- High-level disinfectant was not available during the inspection.-Please provide and use high-level disinfectant such as preempt CS20 or HLD5 if reusing semi-critical equipment such as tweezers and extractors.-If intending to reuse the high-level disinfectant for more than one day, please procure suitable test strip to monitor its concentration and document the test results as required.
