- It was observed that the catippilar indoor play structure does not have adequate fall protection. The structure measures 24 inches, which creates a risk of injury if a child were to fall onto the existing gym flooring. Please ensure that the play structure must not be used until approved indoor playground surfacing is installed and the area is deemed safe for use.

- A client was observed to not complete handwashing after their diaper was changed. The attending staff member was also observed to not wash their hands in-between diapering clients. The facility director was notified of the issue. Please ensure that hand hygiene is performed regularly and in-between diaper changing clients.

- The temperatures of the hot water at several client-accessible sinks in the facility were measured at 54.3-54.7C after running the tap for a couple of minutes at the time of inspection. Please ensure that all client-accessible hot water fixtures are limited to temperatures at or below 54C.
