      • Observation (CORRECTED DURING INSPECTION):
      • Front service and kitchen handwashing sinks do not have a paper towels dispenser found adjacent to the handwashing sinks. Staff are unable to dry their hands after washing them.
      • Corrective Action(s):
      • Immediately install a paper towel dispenser adjacent to the front service and kitchen handwashing sinks. During the inspection, a roll of paper towels were placed at each handwashing sink, until a dispenser is installed.

      • Observation:
      • The high temperature dishwasher is not working as per manufacturer's specification. The temperature gauge dials are broken so staff do not know if the dishwasher is reaching the correct temperatures; the drain plug doesn't stay closed, so inadequate water and soap is present to wash dishes; and the dishwasher is only running one cycle not two. These factors are affecting the dishwasher's ability to sanitize dishes and reach 180F everytime during the rinse cycle.
      • Corrective Action(s):
      • Immediately have the dishwasher repaired and/or maintenanced so it is operating as per manufacturer's specification. There must be 2 stages, wash cycle running at 150F and a sanitize cycle running at 180F. The dishwasher must reach 180F or hotter every time during the rinse cycle. The temperature gauges must accurately read the temperatures during each cycle. During the inspection, manager stated that repair company will be arriving tomorrow to service the dishwasher.

      • Observation: There is accumulated food, grease, dirt, and dust on equipment doors & handles; exhaust canopy (next service May 2022); undersides of sinks and prep tables; legs of tables; and lower shelves of prep tables. These conditions will attract rodents.
      • Corrective Action(s): Immediately wash and sanitize all surfaces with hot water and soap, then sanitize with a bleach and water solution (1 Tbsp bleach per 4L of hot water). Before washing shelves, remove all items off the shelf and then clean it. Failure to clean all surfaces and equipment will result in the closure of the premises, until it is done.

      • Observation: 1) There is no degreaser or soap present on site, for proper washing of surfaces, equipment, floors, and walls. Staff was using Windex to try to remove food and grease. Windex has no grease removing properties.
      • 2) Wiping cloths were being stored at room temperature when not in use. The cloths were stained and very dirty. These cloths will be spreading bacteria and germs when used.
      • Corrective Action(s): 1) Immediately purchase the required products to wash and clean surfaces in the premises.
      • 2) Immediately store all wiping cloths in a bucket of hot water and bleach solution (1 Tbsp bleach in 4L of water), when not in use. When you want a cloth, take one out and use it, then place it back in the bucket. The bleach and water solution must be changed with a new solution every 2 hours.

      • Observation: 1) The front service hand washing sink has no paper towels adjacent to it. Staff are not able to properly wash and dry their hands.
      • 2) The kitchen hand washing sink had items stored in the basin and there was no running cold water exiting the faucet. Staff are not able to properly wash and dry their hands.
      • Corrective Action(s): 1) Immediately install a paper towel holder above/beside the hand washing sink in the front service counter area.
      • 2) Immediately repair the kitchen hand washing sink, so running hot and cold water exits the faucet at all times. Immediately remove all items from the basin and move crates away from the front of the sink, so staff have access to the sink at all times.

      • Observation: Rat droppings found on the floor under the shelving racks in the dry storage room; under equipment and prep tables in the kitchen and front service counter; and on lower shelves of prep tables around cookware. Rat droppings also found on food containers on 2nd and 3rd level shelves in the dry storage room. Rats and their droppings (feces) carry diseases.
      • Corrective Action(s): 1) Immediately clean up all the rat droppings. Wash and sanitize all surfaces, equipment, and cookware that the droppings had touched or were found around.
      • 2) Contact a professional pest company to eliminate the rats.
      • 3) Block all holes and openings in walls and around pipes or electrical outlets.
      • 4) A deep, thorough cleaning of the whole premises from top to bottom is required.

      • Observation: 1) Found holes and openings in walls, corners, around pipes and electrical outlets. Rodents can access the kitchen areas through these holes.
      • 2) General sanitation level of the premises is poor. This is attracting and encouraging rodents to the premises.
      • 3) Mice catch-all-traps are being use, when rats are the problem. The mice traps are too small for rats.
      • Corrective Action(s): 1) Block all holes and openings with wire mesh (1/4" of smaller), metal plates, or tightly pack steel wool in the holes/openings and then fill with expanding foam.
      • 2) Scrub and clean the premises from floor to ceiling to remove all food, grease, dirt, dust, and rodent droppings.
      • 3) Contract a professional pest company, or get a different company, to eliminate the rat infestation.

Remove all items off the floor to clean; get on hands and knees to scrub the floor beneath all equipment and prep tables; crawl under prep tables to clean the walls behind or move all equipment and prep tables away from walls so they can be scrubbed clean.
