    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • Clippers were observed to be taped together. The operator stated that the tape is used to hold the clippers together in their current condition. Therefor, the operator is not adequately assessing the equipment to be used.Corrective Action:- The operator was advised to properly repair or replace the clippers. - The operator confirmed the clippers will be removed from service and replaced.
    • 20. Is reusable equipment disinfected properly?
      • Non-critical equipment (including combs, scissors, and clippers) were not being properly disinfected. Barbicide is not used in accordance with the manufacturer’s instructions. Although Clippercide was available on site, it was not being used. Instead, alcohol was being used which is not an approved disinfectant.Corrective Action:- Operator was advised to properly clean and disinfect this equipment prior to use on the next client.- Operator was educated on proper disinfectant use (e.g. Barbicide and Clippercide), including required pre-cleaning and minimum contact time prior to use as per manufacturer instructions.- Operator was advised that alcohol spray is not an approved disinfectant.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility does not have a written procedure for the decontamination of sink(s) that will be used for both handwashing and equipment reprocessing.Corrective Action:- AHS written procedure template was provided for use.- Please complete and forward copy to me for review.
    • 20. Is reusable equipment disinfected properly?
      • Facility does not have a device to measure the required amount of Barbicide for proper dilution of this disinfectant. The manufacturer instructions on the label state to mix 62.5 ml of Barbicide in 1 liter of water. (This is equivalent to 2 ounces of Barbicide in 32 ounces of water, or 1/4 of a cup of Barbicide in 4 cups of water)Corrective Action:- Ensure you are following the manufacturer's instructions of this disinfectant.- Ensure you have a device to measure the correct volume of Barbicide and water for mixing.

      • Written procedures were completed during the inspection.
    • 20. Is reusable equipment disinfected properly?
      • The one-step disinfectant used to disinfect hair related tools (i.e. brushes, combs, scissors etc.) is missing a DIN on the label. Purchase a low-level disinfectant (i.e. barbicide) - ensure it has a DIN and disinfectant claim.
