    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • During a call with the owner Tina, it was determined that the facility has no written procedures for: skin cleaning/skin preparation, no procedures for accidental cuts, no procedures for cleaning of the foot basins, and no procedures for how to properly reprocess semi critical tools.Please ensure that written procedures for all of the above are available in writing for the next inspection.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The make-up wipes used as a cleansing agent prior to waxing/eyelash tinting was dried out. Requested operator purchase a new product.

      • 1.Single use foam toe separators were found being launder and re-used during the time of inspection. Please ensure that all single use items are disposed of after use.2. At back of the facility and at manicure/pedicure stations evidence of single use items not being discarded after one use was observed.Please ensure that all single use items are disposed of after use.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • A nail nipper at the reprocessing station was found to be visibly dirty after reprocessing.Pleas ensure that workers are doing a through visual inspection and that all equipment is free from visible debris prior to the disinfection process.
    • 20. Is reusable equipment disinfected properly?
      • 1. During reprocessing the technicians are letting the reusable equipment soak in the CS-20 for 10 minutes. Manufacturer's instructions state the equipment must be completely submerged in CS-20 for 20 minutes. Please ensure to allow for the appropriate contact time when reprocessing equipment.2. During the phone conversation with Tina, it was discovered that no soap was being used when cleaning the foot baths at the pedicure stations. Please ensure to wash the foot tubs with soap and water prior to the disinfection step after each client use.
    • 24. Are waste sharps disposed of properly?
      • Sharps container had no lid during the time of inspection. Please ensure that the sharps container is maintained in a secure way as to minimize the risk of injury to the worker.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Scoops for the disbursement of the pedicure food scrub was found inside the bulk container. Please ensure that the scoops are placed outside the tubs in order to protect the bulk supply for contamination.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • General cleaning needed at the nail stations. Nail dust on work surfaces, chairs, and inside the drawers settling on clean ready-to-use items. Please ensure that a general cleaning of the nail stations is occurring on a routine basis.

      • Personal services notification (PSN) form was completed. If you move locations or add/remove services in the future, please let this office know or complete a new PSN.
