- Baby room: the belted diaper pad was not affixed to the diapering table. The pad was also tornReplace with a durable easy to clean diaper pad with a belt affixed direct to the table so that the belt comes up and over the child securing them into place. Floor diapering only until corrected.

- There was a white crib with a mattress. The metal mattress support frame was bent significantly indicating it is not being used as its intended purpose/beyond weight capacity. The other two cribs appeared out of use as they were filled with storage. As per staff all the kids sleep on mats. Repair or discard if it cannot be repaired without alterations

- The walls/trim throughout area damaged/chipped, some were very dirty especially the preschool room. Walls and finishes need to be smooth durable and easy to clean. Make a plan and start fixing the walls.

- The tables in the preschool had tape residue all over them Repair or replace the tables. Children eat at the tables, they need to be smooth durable and easy to clean. Limit the use of tape in the facility on equipment/surfaces.

Chemical bottles under sink, accessible to children in the preschool bathroom(broken child proof lock)- staff removed. Such chemicals need to be out of reach or in locked cabinet. - Dirty rags for surface cleaning were noted throughout. Staff placed in dirty bin If using rags for surface cleaning/disinfecting then must be one per task, then into dirty bin

- 1. Incorrect toy wash routine was described by staff- education provided. 2. Noted dirty cleaning rags out- one rag per task then into laundry bin3. Water play table leaning against toilet in preschool room- keep non toileting items out of the washroom. 4. Paper towel dispensers not working throughout, bulk rolls out. - Repair dispensers so that the bulk roll is not handled especially important where diapering occurs/ washrooms.

- Personal items in toddler room- sippy cups were accessible to youngsters who were touching /taking and drinking from ones that were not theirs. – keep out of reach. More storage needed.
