    • 20. Is reusable equipment disinfected properly?
      • 1. HDL5 testing logs were not being maintained. The last testing was conducted on June 10. Please ensure to test HDL5 and maintain testing logs.2. An intermediate level disinfectant was not observed to disinfect microneedling pen and laser machine. The operator indicated that they use 70% alcohol. Please note that 70% is not considered a disinfectant and acquire an intermediate level disinfectant.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • It was noted that the facility was not following cleaning and disinfection instructions as per manufactures for laser machine. Please use a hospital grade disinfectant to disinfect the laser machine as per manufacturer's instructions for use.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. Stanhedine expired in 2021.2. Open Botox vials in the refrigerator indicated that they are single use and must be discarded after each use. 3. Two saline vials with preservative were expired in 2025. Please discard all expired products. Please follow manufacturer's instructions for use for Botox.4. The staff indicated that they 70% in each room is not the same product. Please properly label all products that are not stored in their original containers.

      • The sharp container was overfilled way above the fill line. Sharp container was replaced during inspection.Ensure sharps containers are filled only to the fill line and discarded safely.

      • - Bacteriostatic solution with preservatives were observed in the injectable room.- Opened and previously used botox were observed in the cooler. No opened date was recorded on the opened vial.- Opened and previously dysport were observed in the cooler. No opened date was recorded on the opened vial.- Opened packs of products termed as sterile and single use was found on the shelves. Ensure 1. single use products are used a single use, as soon as the package is opened.2. Use non-preservative solution as a to reconstitute botox and dysport following manufacturer instructions3. Botox should be used within 24 hours of reconstitution.4. Dysport should be discarded after 24 hours of reconstitution.5. Label your opened product with a date and time.
