    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Single use disposable headpieces for radiofrequency purchased pre-sterilized were being saved for reuse on same client. Several opened packages labelled with client's names were in drawer in back area. Headpieces are labelled as single use and cannot be reused even on same client.
    • 20. Is reusable equipment disinfected properly?
      • Operator had expired bottles of Cavicide and Accel Hydrogen Peroxide Cleaner and disinfectant on-site. Ensure disinfectant onsite is not expired. Do not use disinfectants past expiration date. Alcohol wipes available onsite are not an approved surface disinfectant.
    • 20. Is reusable equipment disinfected properly?
      • Please obtain high level disinfectant for use on semi-critical equipment (Equipment that may puncture skin, such as tweezers used for pulling ingrown hairs).July 23, 2026 - Operator stated she was taking semi-critical equipment home to sterilize in an autoclave. Autoclave and reprocessing area in her home has not been inspected. Obtain high level disinfectant and reprocess semi-critical equipment on-site (microdermabrasion head pieces).

    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Lot number and expiry date of critical equipment was not being tracked with client records. Ensure lot number and expiry date of critical equipment is tracked and can be associated with client information.
    • 20. Is reusable equipment disinfected properly?
      • Please obtain high level disinfectant for use on semi-critical equipment (Equipment that may puncture skin, such as tweezers used for pulling ingrown hairs).
