Operator had plans to paint within the next few days. Ensure all walls are smooth, non-porous and easy to clean.

      • 1. There were water damaged ceiling tiles throughout facility. Replace any water damaged ceiling tiles. 2. Walls in the baby room, OSC room, and hallway had areas of damage. Please repair and re-paint walls in damaged areas.3. Vinyl baseboard was coming off in one area of NW room. Please re-attach.

      • A bottle of Lysol toilet bowl cleaner was kept on shelf in bathroom that may be accessible to some children. The cleaner was moved to an inaccessible shelf.

      • Sleeping mats in baby room were stored touching one another but were not disinfected daily. Though they are dedicated to a specific child, ensure they are disinfected daily to prevent cross contamination between mats.
