      • 1. Clean drill bits and dirty drill bits were stored on different sides of the same tray. Reorganize to ensure greater separation between clean and dirty equipment. Do not store in same area.2. Used single use nail files were stored in container by manicure station. Single use equipment must be discarded immediately after use or properly stored in labelled container for client.

      • Operator had added in head spa without notifying health inspector. Ensure a personal services notification is submitted if any services are added.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Tool used to scrape shellac was dirty and stored at pedicure station. This must be cleaned and disinfected in high level disinfectant.
    • 20. Is reusable equipment disinfected properly?
      • 1. Operator was not tracking results of high level disinfectant testing. Keep log of results to ensure that high level disinfectant is tested every day it is reused during 14 day reuse period. 2. Test strips on-site were expired. Please obtain new test strips. 3. There was a bottle of Pre-empt RTU and a bottle of CS20 that were expired. Discard.
    • 20. Is reusable equipment disinfected properly?
      • High level disinfectant (CS20) in disinfection container had been diluted and failed when tested with test strips. The disinfectant was changed during the inspection, and new disinfectant was tested and passed.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • There was a bottle containing a clear liquid that was mislabeled. The solution was discarded during the inspection. Ensure all bottles are labeled to prevent misuse.

    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Paper towel in washroom were not in dispenser. Place in dispenser to prevent contamination.
    • 20. Is reusable equipment disinfected properly?
      • 1. High level disinfectant (CS20) had been diluted. Operator discarded and refilled the soaking tray during inspection. Do not dilute CS20. Follow manufacturer's instructions for use. 2. Pre-empt RTU was expired. Operator will switch to using alternative disinfectant (Mr. Clean or other disinfectant with DIN) on non-critical surfaces.

      • Operator stated she was soaking tools in CS20 for 5-10 minutes. Tools must be soaked for 20 minutes in CS20 in accordance with manufacturer's insturctions for use.

      • Manicure and pedicure tools for Nails and Hair For you were being disinfected in intermediate level disinfectant (Pre-empt RTU). A high level disinfectant is required for semi-critical tools. The disinfectant was changed out with CS20 (high level disinfectant) during inspection.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Labels had come off bottles of cosmetic products used by Nails and Hair for you. Please ensure all bottles are labelled so their contents are known.

    • 17. Is reusable equipment cleaned properly (Critical)?
      • Tweezers and scissors in waxing room had wax on them and were stored in dirty container with tints. Ensure these tools are cleaned and disinfected with a low level disinfectant and stored in a manner that prevents contamination.
    • 20. Is reusable equipment disinfected properly?
      • No disinfectant log was kept of testing of CS20. Please use log attached in email to track testing.

      • 1. Solution in soaking tray did not appear to be CS20 and did not pass test. The solution was replaced with CS20 during inspection. Ensure manicure and pedicure tools are disinfected by soaking in high level disinfectant. 2. Tweezers used for waxing were being disinfected with alcohol. Ensure at minimum a low level disinfectant with a DIN is used. The operator will switch to using PreEmpt RTU that was on-site.
