- No test strips were available to test the concentration of high-level disinfectant. Obtain the appropriate strips.

- The machines Time Machine Biodome, CB-113 Quick Freckle Eliminator- CME, Hi Frequency and 229-STIM light BSM did not have manufacturer's instructions. Ensure that the manufacturer's instructions are available or discontinue use of the equipment.
