      • A multi-dose vial stored in the cupboard had no record of initial entry. Labelling indicated that the vials must be discarded within 3 days of first use.Record the date and time of initial entry on all multi-dose vials and discard within the required timeframe.

      • The lidocaine cream stored in the mini-fridge in the injector room was expired (April 2025).Ensure all topical anesthetics and related products are routinely monitored and removed from use once expired.

      • - No high level disinfectant was found onsite for extraction tools. Please acquire Preempt CS20 or other HLD for tools. Facility was using Maxill Nextgen wipes (ILD)- Alcohol was being used to disinfect eyewear. Please ensure that Maxill wipes are used for disinfecting surfaces and equipment.

      • Saline with preservative found onsite for reconstitutions. Please change saline to preservative-free.
