      • Three bottles of Pre-Empt CS20 onsite were expired. By not routinely checking for and discarding expired solutions, the Operator and staff run the risk of using expired solution to disinfect tools.REQUIREMENT: Discard all expired solutions promptly to ensure they're not used.

      • 1) Three bottles of Pre-Empt CS20 onsite were expired; there were no bottles of CS20 onsite which were not expired.REQUIREMENT: Immediately obtain a high-level disinfectant (like Pre-Empt CS20), as semi-critical re-usable tools must be disinfected with at minimum and high-level disinfectant.

    • 15. Is reusable equipment handled and stored properly (Critical)?
      • In the permanent make up room, there was a set up with the permanent make up device wrapped in sheathe, ink and pre-sterilized needles laying on a disposable liner on top of a tray. Room has not been occupied since the esthetician had left for vacation. Ensure esthetician put away all of the equipment and supplies when there's no service to be provided to protect these items from possible contamination.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The flooring of the permanent make up room was dirty. Please ensure service provision area is cleaned after each client use. Extended response date to April 2, 2024.

      • Please develop written procedures as outlined in the 2020 Personal Services Standard. A paper copy of the templates was provided at the time.March 6, 2024 inspection: not observed at time of inspection. Please ensure this is readily available during follow up inspection. March 22, 2024 inspection: not completed.

      • Used nail files were found and discarded during inspection. Please do not re-use nail files as these are single use only.

      • Preempt CS20 is kept in the jars for soaking tools for more than 1 day and test strips were not available for monitoring the concentration and a log was not being maintained.-If keeping the Preempt CS20 in the soak tray/jar for more than 1 day, please obtain test strips to monitor the concentration and maintain a log.
    • 20. Is reusable equipment disinfected properly?
      • 1. Nail instruments are disinfected with alcohol and then in RTU.2. CS20 expired in April 2023. 3. The reservoir of the facial steamer was not cleaned with cloudy water.4. Few of the nail instruments were rusted. 1. Nail instruments must be disinfected with high level disinfectant (e.g. CS20). RTU is not a HLD.2. Do not used expired disinfectant as its efficacy is not guaranteed. 3. Clean and disinfect the facial steamer reservoir. Ensure water is changed and emptied daily. 4. Do not use rusty nail instruments.
    • 24. Are waste sharps disposed of properly?
      • Sharps container in the permanent make up room was filled pass the acceptable fill-line. Please replace sharps container and do not dispose pass the fill-line.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The flooring of the permanent make up room was dirty. Please ensure service provision area is cleaned after each client use.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The seat on one of the chairs at the front waxing station is cracked and no longer easy to clean.-Please ensure that client contact surfaces are impervious to moisture and easy to clean and disinfect.
