- Observation(s): The system to ensure a menu is developed for at least a 4 week period was ineffective. It was noted that the menu is currently planned weekly.

- Observation(s): It was identified during the inspection that there is currently no system in place to monitor fridge and freezer temperatures to ensure safe storage of food.

- Observation(s): The system for ensuring that all persons in care have their weight recorded monthly appeared ineffective. This was evidenced by noting that two residents did not have their weights recorded for the month of November.

- Observation(s): Of the records reviewed, all residents had an immunization status on filed beyond flu and pneumovax Although the manager informed LO the day following the inspection that the records had been located, there is lack of a system in place to ensure that influenza immunization records are maintained.

- Observation(s): In review of the residents' records and menus it appears that snacks are not offered daily and when they are offered, they consist of chips and pop.

- Observation(s): The system in place to ensure that medication areas are inspection as per the section 68 of the RCR does not appear to be effective. The pharmacist had reportedly been at the facility within the past year, there was no evidence of the medication storage area having been inspected at that time.

- Observation(s): The residents did not have an "immunization status" other than influenza and TB screening. LO discussed pneumococcal and the Province's immunization program with the manager via telephone after the inspection.

- Observation(s): The emergency plans posted by the door in the living room and the dining area were not updated with the recent changes which were made to the copy in the office.

- Observation(s): One resident has not had a nutritional/meal audit completed since 2011. In addition, the record of food consumed by residents is not being consistently documented making it difficult to assess nutritional intake, changes in habits etc.

- 10.3 Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
- Observation(s): Three residents' oral care plans were noted to be overdue for review/revision. A thorough medication care plan was noted on the inside door of the medication cupboard, however, the care plan is over due for review as it is dated 2012. Some of the nutritional care plans/assessments had not been reviewed since February of 2014.
- Review and, if necessary, modify each care plan if there is a substantial change in the circumstances of the person in care, or at least once a year, to ensure it continues to meet the needs, preferences, and is compatible with the abilities of the person in care; 81( 4 )(b)(i)(ii)
