- Facility specific written procedures for services provided int he facility were not completed. AHS templates were provided to be completed. post care instructions were reportedly available. - Re-useable towels were noted being used in the washroom of the facility. Provide single use microfiber towels in sufficient quantity with basket to collect used towels in the washroom or provide paper towels. Store towels in a way to prevent contamination from the splashes from hand washing.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Sterility assurance documentation for pre-sterilized equipment such as microneedling, dermaplaning was not recorded with client records. Discussed with operator and advised to keep the label (lot #, batch number and expiry date) of cartridges, blades, with client records.

- 1-Semi-critical tools were noted not being immersed in the high-level disinfectant. Operator was educated on the importance of fully immersing the tools in high level disinfectant.Ensure tools are fully immersed in high-level disinfectant for it required contact time.2-High level disinfectant daily testing log was not adequately maintained. The daily testing log was reviewed during inspection and advised operator to use sheet for one pour of high level disinfectant and in the event of a fail test or end of 14 days re-use period begin with new sheet and start with day one (new pour day).Ensure daily testing results are correctly logged in the daily monitoring testing log.

- Operator knowledge about the contact time, and re-use period of high-level disinfectant was noted being limited. Contact time, re-use period and manufacturer instructions for use of a disinfectant were discussed during inspection onsite.Ensure the manufacturer instructions for use of a disinfectant is fully reviewed and understood prior to its use. Follow contact time to achieve adequate disinfection of the semi-critical tools.

- 1-Manuals for hydra-facial, ultrasonic cavitation equipment were not readily available during inspection. Obtain the manuals for all equipment used in the facility and ensure these are accessible to operators.2-Bacteriostatic saline was being used to reconstitute Botox injections. Obtain preservative saline solution to reconstitute Botox injections.

- 1-Dermal filler injections were noted being used in contradiction of "manufacturer instruction for use". Injection was noted being dispensed using single use syringes to serve more than one client. "Manufacturer instructions for use" from package were reviewed with operator and advised to use one package for one person as mentioned on the package.2-Botox vial was not labeled for its opening date to track 24 hours use period. Operator was advised to keep log of Botox vial and label the bottle to ensure the remaining content is discarded at the end of 24 hours period.

- 1-There was no thermometer to monitor the temperature of fridge where Botox injections were stored. Ensure temperature of the cooler is monitored and a thermometer is placed in the cooler.2-Wax container was noted in unsanitary condition. Wax spill was noted all over the surface of wax warmer equipment. Lids were sticky. Operator reportedly planning to replace the wasx warmer equipment.
