      • The handwash sink located in the Junior Movement Room (washroom) was observed to be slowly draining.Please repair the sink to ensure they are adequately draining.

      • The chlorine disinfectant in the washroom located in the Senior Movement room was tested and measured at 100ppm. During an outbreak the disinfectant concentration must be increased to 1000ppm to control the spread of illness.The operator was instructed to replace the 100ppm chlorine disinfectant bottle with one measuring at 1000ppm.

      • 1) The handwash sinks located in the Junior Movement Room (washroom), and the Senior Left Brain Room (short handwash sink) were observed to be slowly draining.Please repair the sinks to ensure they are adequately draining.2) The tall handwash sink located in the in the Senior Left Brain Room was observed to be chipped exposing sharp edges.Please repair the handwash sink.

      • Right Brain Senior Room: Playdoh was stuck in carpet fibers. Deep clean carpets. Junior Movement Room: Floor in movement room was visibly soiled. Clean and disinfect indicated area.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Long tangling wires were observed in senior preschool and left-brain classrooms.-Please properly secure the indicated wires or keep long wires out of reach of children to prevent risk of entanglement.
    • 06. Are proper protocols being followed in regards to client care?
      • Update 21/Feb/2025: There was no diapering was not observed at the time of inspection.The handwashing procedure after diaper changes was not properly followed or managed. Children's hands were not washed immediately after diaper changes, and it was observed that children were putting their unwashed hands into their mouths after touching the diaper pail and other surfaces in the washroom. This occurred while caregivers were busy clearing the diapering surface and area. This infraction was noted in two separate classrooms: the baby room and the movement room.Action Required:-Review diaper handwashing procedures with all staff members.-Ensure children's hands are properly washed immediately after diaper changes and that they are either led out of the washroom or handed over to another staff member.-Staff members must also properly wash their hands after clearing and cleaning the diapering surface and area.

      • Toy sanitizing was being done in the order of Wash, Sanitize, and Rinse.*Explained the correct 3 step method, which is Wash, Rinse, and Sanitize with 100 ppm of chlorine for 3-5 minutes (when not in an outbreak) followed by air drying. During an outbreak, an additional step of rinsing again (4 step) is required, as the bleach concentration used to sanitize the toys is 1000 ppm, which may remain on the surface.*Staff will resanitize all the toys in the classrooms.

      • - One of the diapering mattresses was missing. Staff explained that the safety belt was in disrepair. Additionally, the station was not clean and had dust and debris present.*Please ensure that the station is maintained in a clean and sanitary manner at all times.

      • The temperature of the hand washing sink was found to range between 7.7°C and 53°C.*Please ensure the water temperature is warm enough to practice proper handwashing (recommended temperature is 49°C). The staff has submitted a maintenance request to have this issue corrected.

      • Update 21/Feb/2025: Inspection was conducted after mealtime and didn't observe staff using gloves.Gloves were being used during meal time. Observed 3 staff members wearing gloves and doing various tasks - feeding, cleaning, and moving children - without washing hands and changing gloves. Gloves must be removed after each task, hands will be washed with soap and water, dried, before new gloves are put on.*Outstanding violation.

      • Update from 21/Feb/2025: Sensory materials were not observed in the classroom as they have been removed by staff due to the ongoing outbreak.2. Uncleanable sensory materials not being changed weekly.-Please ensure to replace uncleanable sensory materials regularly. All uncleanable sensory materials must be exchange for new ones weekly to prevent unsanitary condition.

      • 1. Long tangling wires were observed in senior preschool and left-brain classrooms.-Please properly secure the indicated wires or keep long wires out of reach of children to prevent risk of entanglement.2. Glass jars were readily accessible to children in senior preschool classroom.-Please keep glass containers out of reach of children to prevent injury in an event of breakage. Such glass jars can only be used in the classroom under staff supervision.

      • The waste receptable in baby classroom could not be opened using the foot pedal.-Please repair or replace the indicated waste receptable so that children or staff can open it without having to use their hands.

      • Sr Preschool gym room: after lunch, 4 children were observed leaving the bathroom without adequately washing their hands. Ensure children are performing adequate handwashing after using the bathroom.

      • 1. The flooring in the Jr movement room has a build up of dirt and grime.2. The flooring in the Jr right brain room has a build up of dirt and grime.Ensure these floors are thoroughly cleaned. Flooring may be prone to build up due to the rubberized surface. 4. Play mats worn and have holes in them in the jr left brain room. Repair so that it is smooth and easily cleanable or removed.5. Couch in jr left brain room is worn and has holes in it. Repair so that it is smooth and easily cleanable or removed.*Nov 23 2023 - floors have been cleaned but still have a build up. A second thorough clean will be completed. If cleaning is not possible floor may need to be replaced. - Play mats have been repaired with a tape on pleather but it is peeling. These will need to be replaced.

      • A mop and broom stored in classroom in sr left brain small room. Ensure these are stored away from children to prevent cross contamination of dirt or chemicals from mopping.
