      • The laundry room and kitchen doors currently lack locking mechanisms, which may allow children to access these areas unsupervised.Install a locking system on the laundry room and kitchen doors.

      • Expired Benzalkonium Chloride wipes were noted in a first aid kit.Chek all first aid kits in all rooms and replace all expired items.

      • Two screws on a crib in the baby room were found to be improperly secured.Ensure all screws on cribs are fully tightened and securely fastened.

    • 06. Are proper protocols being followed in regards to client care?
      • A staff member in the toddler room did not disinfect the changing pad after changing the diaper of a child. Upon observation, the staff member was asked to disinfect the pad.Please discuss proper diapering procedures with staff and ensure the diaper changing pad is disinfected between children.

      • The cover on the couch in the preschool room is visibly dirty/stained.

      • The diaper changing pad in the toddler room has rips in it. Please replace the pad.

      • The couch in the kinder room has multiple rips in the cushions and is being held together with duct tape. Please either dispose of the couch or purchase a cover for it that can be laundered regularly.
