    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures are not available for the following Practices. - client care in the event of an accidental skin cut or puncture.- decontamination of any sink that will be used for both handwashing and equipment reprocessing.Documents were left onsite for operator to complete written procedures. Digital copies provided with report as well.
    • 20. Is reusable equipment disinfected properly?
      • When inquired, inspector was informed that high-level disinfectant is same solution dispensed as day before.Test strips for high-level disinfectant, and a high-level disinfectant log is not available.Obtain test strips that are required by manufacturer of high-level disinfectant that is being used.Demonstrate knowledge of use of high-level disinfectant, test strips, and disinfection log.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Ingredient lists and some manufacturer's instructions were not available for all cosmetic products used in the personal services facility including but not limited to nail polish and brow tint.Ensure that a copy of ingredients and manufacturer's instructions are kept onsite.Facility will obtain manufacturer's instructions from websites, or retailers to ensure a list of ingredients are onsite. Products observed were routine for services offered.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Visible debris is observed on the following areas but not limited to:- In the bottom and on the top of the UV chamber used to store clean equipment.- On the towel that is under the towel warmer.- On the surfaces of products, equipment and cosmetic containers and shelving in the manicure area. - At the bottom of containers used to store cosmetic products.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The surface that the towel warmer is placed on is visibly water damaged, and no longer smooth, non-porous and easy to clean.The nail desk drawers were lined with felt fabric that are not easy to clean and disinfect.Repair or replace the surface that the towel water is stored on, so that it is smooth, non-porous and easy to clean and disinfect.Remove or repair liner of the nail desks drawers so that they are smooth, non-porous and easy to clean and disinfect.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • There is an electrical outlet cover missing in the laundry room.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Cooking of personal foods was observed in the laundry room of the facility.Ensure that non-compatible activities are not occurring in areas that procedures for the facility occur. I.e. separate food from laundry room. On the re-inspection inspector discussed moving foods to the room where services are not offered. Cooking and re-heating was not observed during re-inspection. Only storage of packaged foods.

      • Personal Services Notification has not been received by Regional Health Authority (Alberta Health Services).
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures are not available for the following Practices. - client care in the event of an accidental skin cut or puncture.- decontamination of any sink that will be used for both handwashing and equipment reprocessing.
    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • Disinfectant Manufacturer's instructions are not being followed. Disinfectant is being mixed with a cleaning product.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Used dremel bits are being stored with clean dremel bits. Store clean and contaminated equipment separately.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Equipment that was "reprocessed" had visual contaminating still present, namely, the inside of nail clippers had visible residual from cleaning products. Ensure that re-usable equipment is cleaned, dried, and visually inspected prior to disinfection.
    • 20. Is reusable equipment disinfected properly?
      • 1. A high-level disinfectant was not being used to reprocess semi-critical instruments. Use a high-level disinfectant on all re-usable semi-critical equipment. 2. High Level-disinfectant was being mixed with cleaning tablets. 3. A log of high-level disinfectant use was not being kept.4. Test strips for high-level disinfect were not available.

      • Manufacturer's instructions and ingredient lists were not available for all cosmetic products used in the personal services faiclity. Ensure that a copy of ingredients and manufacturer's instructions are kept onsite.

      • Nail dust was observed on the following items and surfaces but not limited to:- Nail employee chairs,- Equipment Carts.- Equipment in the equipment cards. and other surrounding areas. - Non-cleanable surfacing was used to line the drawers of the nail desks. Ensure all surfaces are non-porous, easy to clean and disinfect. Ensure all surfaces and equipment is routinely cleaned and disinfected.

      • Written Procedures are required for hand sink/ reprocessing sink to ensure proper disinfection occurs between different uses.
