      • 1. Hand sanitizer expired in 2022. The hand sanitizer was discarded during the inspection. Ensure that hand sanitizers used have NPN/DIN, alcohol content between 60 and 90%, and are not expired. 2. Alcohol (no NPN or DIN) was used for hand hygiene and surface disinfection. Ensure that an approved hand sanitizer is used for hand hygiene, and approved disinfectant is used for surface disinfection.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Antiseptic product used for skin preparation expired in April 2025. Please obtain new antiseptic product.

Barbicide disinfectant was used against manufacturer's instructions: it was sprayed on surfaces (cannot be sprayed) and used for tweezers disinfections (must use a high-level disinfectant).
