      • The owner of the personal service business did not submit a notification about the business to the Alberta Health Services. Submitted during the inspection.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures for skin cleansing prior to services, antiseptic use, and accidental cuts were available.
    • 04. Are business-specific written procedures established to adequately describe safe and sanitary practices in a mobile business?
      • No written procedures for mobile services were available.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • 1. Hand sanitizer expired in 2022. The hand sanitizer was discarded during the inspection. Ensure that hand sanitizers used have NPN/DIN, alcohol content between 60 and 90%, and are not expired. 2. Alcohol (no NPN or DIN) was used for hand hygiene and surface disinfection. Ensure that an approved hand sanitizer is used for hand hygiene, and approved disinfectant is used for surface disinfection.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Antiseptic product used for skin preparation expired in April 2025. Please obtain new antiseptic product.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation for sterile equipment was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms.
    • 20. Is reusable equipment disinfected properly?
      • 1. Preempt RTU disinfectant expired in 2022. Ensure that the disinfectants are used as per manufacturer's instructions. 2. Barbicide disinfectant was used against manufacturer's instructions: it was sprayed on surfaces (cannot be sprayed) and used for tweezers disinfections (must use a high-level disinfectant).

      • Lumaiux Plus machine did not have manufacturer's instructions (light). Ensure that the manufacturer's instructions are available or discontinue use of the equipment.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. Some pigments were expired. Discarded during the inspection.2. Numbing cream was used against manufacturer's instructions: numbing creams expired in March 2024 and April 2025. Obtain new numbing cream.3. Some needles used for permanent make-up services were used against manufacturer's instructions: they expired in April and March 2025.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Some cosmetic products did not have appropriate labels. Ensure that all cosmetic products have appropriate labels.
