- There are single-use nail files, sanding drill heads, and buffers soiled with debris and was being reused. Items that are listed as single use should not be reused more than once. Discard the single-use item after use.

- There was debris lodged in the crevices of several nail clippers and cuticle clippers.Reusable equipment and tools must be thoroughly cleaned and visually inspected prior to disinfection. Ensure the tools are cleaned, use a brush to dislodge debris in the crevices. Visually inspect the tools prior disinfection.Observed proper cleaning of the cuticle clippers and nail clippers during the inspection.

- There is rust observed on several callus clippers. Equipment and tools must be maintained in good repair to ensure proper cleaning and disinfection. Visually inspect equipment and tools and discard once they are rusted.
- 20. Is reusable equipment disinfected properly?
- 1. No high-level disinfectant available. There was a Preempt CS20 high-level disinfectant and it was expired. A high-level disinfectant must be available at all times to ensure semi-critical tools are adequately disinfected. The operator obtained a high-level disinfectant (Preempt HLD5 and CS20) during the inspection. 2. There was a spray nozzle on the Preempt CS20 high-level disinfectant. The operator stated they were spraying semi-critical tools with the high-level disinfectant instead of fully immersing them in the solution. The manufacturer's instructions must be followed to ensure semi-critical tools are adequately disinfected. The operator read the instructions and performed proper cleaning and disinfection for the semi-critical tools. 3. The accelerated hydrogen peroxide 7% test strips for the Preempt CS20 high-level disinfectant was expired. The expiration date was overwritten with pen to indicate a later expiration date. The test strips expired in 2024. The operator obtained accelerated hydrogen peroxide 2% test strips for the Preempt HLD5 during the inspection.

- There were several hair strands observed in both wax pots used for the face and the body. Hair strands were also observed in the bulk supply of the petroleum jelly.When portioning the bulk supply, it must be done in a manner that protects the bulk from contamination. That is, using a clean tool to remove a portion of the supply and refrain from "double-dippng".The operators discarded the petroleum jelly and the wax pots during the inspection.

- Several cosmetic products were decanted into another container and did not have a label. Cosmetic products must be properly labeled for staff awareness and safe usage of the product. The operator relabelled the containers during the inspection.

- 1. There is dust and debris throughout the facility and client surfaces. Including but not limited to the client bed located in the wax room, counters for manicure, and inside the drawers. The facility and client surfaces must be maintained in a clean and sanitary manner. Ensure regular cleaning is performed to remove dust and debris.2. There is wax debris on the wax cart. Thoroughly clean and disinfect the area.

- Operator used nail glue to close a wound from a customer. Ensure to use an appropriate antiseptic for wounds. Recommend customers seek medical attention if this occurs.

- Some of the foot razor tools are rusted. Ensure these are discarded.

- Facility was using Pre-empt rtu to disinfect tools. Switch to CS20 onsite. Ensure all staff are trained on adequate tool cleaning and what disinfectants to use.

- The bulk supply of foot scrub is being contaminated by reusing spoons. Ensure spoons are one time use and then cleaned/disinfected.

- 1. CS20 solution measured 0ppm. Operating was adding water to solution and only partially changing the solution. Ensure manufacturers directions are followed. Do not add water. Discard whole solution when finished.2. Facility is not testing or tracking daily high level disinfectant. test strips not available. Obtain test strips. Track and log high level disinfect concentration daily.
