    • 20. Is reusable equipment disinfected properly?
      • The test strips for the Preempt CS20 high-level disinfectant was expired. Disinfection logs were completed; however, the test strips were expired and the solution in use was a Preempt concentrate. Therefore, the tested solution cannot produce accurate results, and the record log was falsified. Test strips are required to determine if the concentration of the high-level disinfectant solution is effective to reuse. Obtain appropriate test strips depending on which high-level disinfectant is used. Ensure all staff is aware of which test strips to use for the high-level disinfectant.

    • 20. Is reusable equipment disinfected properly?
      • 1. No high-level disinfectant was available. The operators were using Preempt concentrate that says it cannot be used on semi-critical tools. A high-level disinfectant is required to disinfect semi-critical tools that come into contact with mucous membranes. Please obtain. Ensure the disinfectant has a high-level disinfectant claim, a DIN or MDL, expiration date, and manufacturer's instructions for use. 2. The test strips for the Preempt CS20 high-level disinfectant was expired. Disinfection logs were completed; however, the test strips were expired and the solution in use was a Preempt concentrate. Observed the operator place wet tools directly in the disinfectant after cleaning. Tools and equipment must be thoroughly dried prior to placing in the high-level disinfectant. Traces of water can dilute the high-level disinfectant solution and decrease its effectiveness and complete disinfection cannot be accomplished thus, increasing the risk for infection transmission. Ensure the tools are dried prior to placing inside the high-level disinfectant solution.
    • 20. Is reusable equipment disinfected properly?
      • 1. The following tools/equipment were disinfected with alcohol: - Metal drill heads used for manicure - The oxygeneo machine - The lash tweezers - High-frequency wands- Microdermabrasion equipment Non-critical tools must be disinfected with either a low or intermediate disinfectant.Ensure all equipment is classified correctly (non-critical, semi-critical, critical) and determine the appropriate disinfectant to use. - Non-critical tools/equipment: does not come into contact with mucous membranes but comes into contact with intact skin or hair requires a low-level or intermediate-level disinfectant. - Semi-critical tools/equipment: comes into contact with mucous membranes and requires a high-level disinfectant. - Critical tools: Punctures the skin or enter sterile tissues requires steam sterilization.2. The operator stated they are reusing the barbicide for more than a day. There is visible debris inside the solution. As indicated on the manufacturer's instructions, a "fresh solution must be prepared daily or more often if solution becomes diluted or soiled". Ensure the manufacturer's instructions is followed to achieve disinfection.3. The impeller for the pedicure basin had buildup of debris. The impeller and the pedicure basin itself must be adequately cleaned and disinfected to remove contamination and prevent the growth of biofilms. Ensure the impeller is taken apart and cleaned to remove all debris. The pedicure basin and its components must be disinfected with a low or intermediate disinfectant.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • There were several hair strands observed in both wax pots used for the face and the body. Hair strands were also observed in the bulk supply of the petroleum jelly.When portioning the bulk supply, it must be done in a manner that protects the bulk from contamination. That is, using a clean tool to remove a portion of the supply and refrain from "double-dipping".
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • The operators are using LeedFrost cream Lidocaine 10.56% and it does not have a DIN/NPN. Lidocaine must have an active DIN/NPN. Lidocaine without a DIN/NPN is prohibited to use. Obtain a lidocaine with a DIN/NPN.

    • 20. Is reusable equipment disinfected properly?
      • Facility does not have a high level disinfectant, test strips or logs. Ensure a high level disinfectant is purchased and concentration is tracked daily.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Microblading sterility information is not document. Ensure lot and expiry or manufacturers date is tracked with client records.
