- - Unexpected eggs were observed in the walk-in cooler. The operator was promptly notified and advised to remove them immediately. The operator was further advised that uninspected eggs are not permitted to be stored or sold in an AHS-permitted facility.

- Current: Mouse droppings observed in the dry storage area. April 2026 pest control monitoring indicated the presence of a dead mouse on the monitoring station under the front bar sink. Previous (December 9, 2025): Mice activity in the furnace room was noted on the pest control report for November 2025. Report indicates there is feeding activity noticed from the bait stations.

- The following areas were observed to require cleaning:(X) In the storage room, there is an accumulation of dry food debris.(X) Moused dropping observed in the storage room on the floor near the shelving unit. (X)The grease trap adjacent to the dishwasher requires cleaning, along with the surrounding floor area.(X)Debris accumulation was observed beneath the dishwasher.(X)In the food preparation area, dry crumbs and debris were noted.(X)An accumulation of grease and debris was observed behind the grill and deep fryer.Overall, the facility requires a thorough deep cleaning to ensure clean and sanitary conditions are maintained and to support safe food preparation practices.

- (X) Bleach concentration is 1000+ as indicated on the chlorine test strip. PHI advised the operator to remake the solution and to aim for a concentration of 100 ppm to minimize the risk of excessive chemical residue and chemical contamination to food items. The solution was remade immediately.

- (X) Multiple food items in the walk-in refrigeration unit are not being covered (e.g, shrimp, and vegetables.) The operator has been advised to ensure all stored food are covered to minimize the risk of contamination.

- - Various foods, such as whole fish and containers of cut vegetables, were found uncovered in the walk-in refrigeration unit. The operator was notified, and the uncovered food products were covered immediately.

- - The commercial dishwasher is not reaching the required high temperature for proper sanitization of dishware and utensils. Additionally, there is no detergent connected to the dishwasher or available in the facility. Please make sure to service the dishwasher and get detergent to ensure that dishware and utensils are properly cleaned and sanitized.The two-compartment sink will be used for dishwashing until the dishwasher is repaired.

- Current: March 19, 2025:- Mouse droppings have been observed in the back storage area, and it was noted that the operator has not procured pest control services since the last inspection. To prevent further infestation, please ensure that pest control services are obtained immediately. Previous: Evidence of pest activity during inspection. The follow areas were observed to have evidence of rodent droppings.- Back storage room -Behind ice machine-Foods stored in the facility are all covered and protected from pest access.-Receiving door does not show signs of light, does not provide access to pests.ACTION: Ensure pest control company is notified of the activity as soon as possible. Consult pest control on possible other areas of access for pests. Seal all areas indicatedEnsure rodent droppings in the areas indicated are cleaned using bleach water spray before sweeping.

- 1) Ice scoop stored in bin above ice machine. Scoop visibly clean, bin visibly unsanitary. Operator placed both items in the dishwashing area and submerged in soapy water.ACTION: Ensure that the scoop and container was washed and sanitized daily.2) Knives in the knife block insert (at counter level) observed to be visibly unsanitary with food debris on the blades and handles. Operator removed the knives and placed them in the dishwashing area to be cleaned and sanitised.ACTION: Ensure that utensils and equipment are stored clean and sanitary.

- The floor under the deep fryer has a significant build-up of grease, dirt and food debris that can act as a potential stockpile of food for pests. This area should be cleaned as soon as possible and should be cleaned more regularly moving forward to prevent this build-up. Dec 13: Some work has begun on cleaning this area of the floor. The operator informed the inspector that the floor would be cleaned the following day.

- Cleaning cloths were observed laying on counters, food prep areas, etc. Upon request, the operator moved them into the sanitizer bucket which was measured at 100ppm chlorine. Always ensure cleaning cloths are stored submerged in sanitizer solution when not in use.

- Various foods were observed to be uncovered in the walk-in cooler. The operator covered some foods with plastic wrap during the inspection but left some uncovered. Please ensure all foods are covered while in storage.
- 10. Are food temperatures during storage and processing being controlled to maximize food safety?
- The cooler drawers were measured at 8C. Upon request, the operator was able to lower the temperature of the cooler, rapidly dropping the temperature to 2C. All coolers must be held at 4C or below in order to keep food safe to consume. It is recommended to start tracking temperatures daily with a temperature log.
