      • Personal Service Notification was not submitted after the facility moved its location.Requirement for PSN were discussed, PSN was submitted during the inspection.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?

    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Personal service workers were maintaining the client records, however sterility assurance documentation for the dermaplaning (bought sterile at point of use) were not being maintained.Please make sure that you note the Lot# of dermaplaning blades along with the client record.
    • 20. Is reusable equipment disinfected properly?
      • Client was using the pre-empt CS20 (HLD) for semi-critical instruments.Re-use period of Pre-empt CS20 was 14 days.Test strips were not available for testing of HLD.Logs of HLD were not available.Discussed with operator the requirement for daily testing of HLD.Based on choice of disinfectant AHP7 test strips are required.Logs for HLD were provided during the inspection.
