      • - 1. Pink Room: Chemicals were stored in the cabinet under the staff sink, and the child safe lock was not secured to prevent access. **Please ensure the cabinet is locked to prevent children access. ** This was corrected during the inspection. 2. Strategist Room: The Lepage glue stored in the cabinet under the staff sink did not have an AP label and stored in an accessible cabinet. ** Please ensure the cabinet is locked and ensure all glue/ paints/ craft supplies used by children are non-toxic and have an ASTM label or AP Label. ** The glue was disposed of at the time of the inspection and the cabinet locked - CORRECTED. -
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • - 1. Kindergarten/Pink Room: There was a metal piece protruding outwards towards the play area from under the baseboard heating units. ** Please eliminate hazard by readjusting the metal piece away from the play area or covering the area / making inaccessible. 2. Strategy Room: The electrical outlet plug cover was missing beside the sink. ** Ensure the child safe cover is installed over the outlets. **This was corrected during the inspection. 3. Strategy Room: The shelf measuring greater than 4 ft high, located beside the desk was not secured to the wall. **Secure the shelf to the wall so it does not tip over and injure kids. 4. Therapy Room: The electrical outlet plug cover was missing. ** Ensure the child safe cover is installed over the outlets. **This was corrected during the inspection. -

      • - Purple Room: The ceiling fan in the bathroom was water stained around the fan. ** Determine the source of water or moisture causing the water staining and properly repair. Then, remove the ceiling tile and replace with new materials.-
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • - Green Room: There was no soap available at the staff handwash station. ** Ensure soap is available at the handwashing stations at all times. **This was corrected at the time of the inspection. -
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - 1. Kindergarten / Pink Room: Brown stains were observed on the black colored carpet in the room. ** Please properly clean and sanitize the area. ** Discussed blood/bodily fluid clean up procedures with the operator. 2. Kindergarten/ Pink Room: Some foam blocks were in disrepair with bite marks/ missing sections. ** Please remove the blocks that are in disrepair. **This was corrected during the inspection. 3. Kindergarten/ Pink Room: The keyboard for children play was missing some keys and the electronic components were exposed. ** Please remove the broken keyboard for play. **This was corrected during the inspection and replaced with a new one that was in good repair. 4. Kindergarten/ Pink Room: the Toy truck with the long string at the end of it may be a choking hazard. ** Please remove or shorten the string so it is not a choking hazard. ** This was corrected during the inspection.5. Purple Room: The diapering mat was stored directly on the floor beside the toilet in the washroom. ** Please relocate the diapering mat so it is stored off the floor beside the toilet to prevent contamination. **This was corrected at the time of the inspection.=

      • Hot water temperature was measured between 33 and 34 degrees Celsius at most washrooms.-Please increase and maintain hot water temperature between 38-43'C to ensure proper handwashing practices.

A table with rough and peeling surface was observed in the extension room.-Please repair the damaged table to ensure proper clean and sanitizing of all surfaces.
