      • The owner did not submit a Personal Services Notification about the business to AHS.

      • No written policies were available for skin cleansing and accidental cuts.

      • Reusable cloth towel was used for hand drying at a hand sink. The hand sink must be supplied with paper towels or single-use towels.

      • Combs were not washed prior to disinfection - Barbicide containers had hair. Ensure that combs are washed prior to disinfection, and the solution is changed at least daily, as per manufacturer's instructions.

      • No disinfectant was available for surface disinfection, and hand sanitizer was reported to be used for surface disinfection. Obtain an acceptable disinfectant.
