Sterilization times and exposure times do not match the packaged and unpackaged cycles from each of the sterilizers.* External and internal chemical indicator on packages were not documented in the sterilization log.* Biological indicator results were not documented in the sterilization log.Please ensure:- Physical parameters are accurately documented. (Statim 2000 - unpackaged, 3.5mins at 135C / Ritter M9D - unpackaged, 3 mins at 270F, packaged 5 min at 270F)- Verify that all packages have their internal and external chemical indicators have passed and document those results in the sterilization log.- BI results are documented in the sterilization log.
