      • **December 16, 2025 - Issue remains in non-compliance. Operator is reportedly obtaining supplies from a new supplier, and is due to arrive tomorrow.** A bottle of PREempt RTU in the reprocessing area was expired. No unexpired bottles were available onsite.- Please discard and obtain unexpired ones.Ensure that only unexpired products are in use according to manufacturer's instructions.Use PREempt RTU to disinfect surfaces. Discontinue the use of CS20 for surfaces (only use for disinfecting semi-critical equipment)
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Accumulated debris was noted in the magnetic impeller of the pedicure chair foot basin.- Please clean and disinfect.- Ensure to disassemble, clean and disinfect the magnetic impeller in the foot basin of the pedicure chair after every use, to prevent harboring microorganisms and increasing transmission risk between clients.

      • Linen used to cover massage bed in the facial room was worn out.- Please replace.Ensure that surfaces which clients come in contact with are easily cleanable and in good condition.

      • Single-use items such as nail files and buffers were noted in the reprocessing area.- Please discard.Ensure that disposable/single use items are discarded immediately after use to prevent potential cross-contamination. Follow manufacturer's instructions for reprocessing items specified as re-usable.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Semi-critical equipment such as foot files, nail clippers in the clean reprocessed area were observed to have visible debris.- Please clean thoroughly and repeat reprocessing/disinfection.Ensure to properly clean and visually inspect equipment before disinfection to prevent cross-contamination.

      • The high-level disinfectant did not meet manufacturer's testing requirements as there was a delayed test-strip reaction time of approximately 55 seconds instead of 35 seconds.- High level disinfectant was discarded and replaced.Ensure that high-level disinfectants are maintained according to manufacturer's instructions. Avoid accidentally or intentionally diluting with water.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility specific written procedures were not in place. Complete the following written procedures and ensure they are easily accessible for all staff. 1. The cleansing of skin and mucous membrane. 2. Decontamination of any sink that will be used for both handwashing and equipment reprocessing.
    • 20. Is reusable equipment disinfected properly?
      • Pre-empt RTU is used to disinfect semi critical tools. Please ensure a high-level disinfectant is used to disinfect semi-critical tools. Ensure to follow the following steps for cleaning/disinfection:- Clean tools using water and detergent. Rinse and dry thoroughly. - Pour high level disinfectant (example: PREempt™ CS20) directly into soaking tray. Do not dilute.- Fully immerse tools in high level disinfectant. RINSE, DRY & STORE- Rinse tools with water and dry manually using clean cloth or disposable towel. Store in a clean container marked “DISINFECTED”
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Some of the cosmetic products such as eyelash cleanser, Damman debonder, primer, lotions do not have adequate labelling (labelling is in Chinese). Please ensure all cosmetic products have adequate labelling to disclose a complete list of ingredients and/or manufacturer's instructions for safe use.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Vinegar solution is used to disinfect pedicure basins. Please ensure at least a low-level disinfectant is available to disinfect surfaces and pedicure basins. Corrected onsite. Pre-empt RTU is available to disinfect the surfaces and pedicure basins.
