- A personal services vendor notification was not received. Please be advised to submit a personal services notification to AHS at least 30 days prior to the event.

- A hand sanitizer or hand washing station was not present at the vendor booth. The operator indicated that they would use washroom hand washing sinks for hand washing purposes. Please acquire an alcohol-based hand sanitizer with minimum of 60% alcohol content along with DIN or NPN number.

- 1. A disinfectant to disinfect work surfaces was not observed. Please acquire a disinfectant. 2. A disinfectant to disinfect hair tool was not observed. The operator indicated that they would get barbicide to disinfect hair tools.
