- Verbal post-service care instructions were provided to clients after ear-piercing, but not in writing. Please provide clients with post-service care instructions both verbally and in writing after the service is complete for ear-piercing.

- Personal services workers provide client records, but they do not contain sterility documentation (needle Lot #) attached to each client record. Education was provided by the PHI trainee on why this would be necessary.Please ensure that client records contain sterility information (needle lot numbers) attached to each client's records, as well as their full name, contact information, date of service, description of service, and employee who performed the service.

- Used piercing gun and cartridges were stored on the clean towel area used to store the antiseptics and the previously unused gun. After providing education on the importance of keeping clean and used equipment separate by the PHI trainee, staff removed the gun, discarded the cartridges and proceeded to disinfect the gun.Ensure dirty items are not stored with clean items.

- Used piercing gun and cartridges were stored in the clean bin with the antiseptics.Staff removed the guns, discarded the cartridges and proceeded to disinfect the content of the bin including the bin. Ensure dirty items are not stored with clean items.

- Used piercing guns were observed. Staff indicated piercing gun was cleaned with alcohol antiseptics in the past. Staff planned to use the available Lysol for disinfecting during inspection.Ensure;1. piercing guns are disinfected immediately after use.2. a low-level disinfectant like Lysol spray or wipe is used to disinfect the tools after each use, and the required contact time should be maintained.
