    • R4.1 - Are person in care records current, complete and kept confidential?
      • Observation(s): Weight records for persons in care were reviewed at random. Some monthly weights were not recorded, nor were refusals to be weighed indicated. The Residential Care Regulation requires that each person in care be weighed monthly and a record of this be kept in order to ensure staff are aware of any significant fluctuations that need to be addressed. Licensing must receive written confirmation that corrections have been made no later than November 12, 2025.
      • R4.1U - Weigh each person in care monthly and record their weight in their nutritional plan (Does not apply to Hospice); 83( 4 )(a)(c)
    • R4.4 - Are records kept on each employee with the necessary requirements?
      • Observation(s): Employee records were reviewed at random. Evidence of reference checks could not be located as part of all records reviewed. The residential care regulation requires that a licensee keep employee character references and provide evidence of these checks to ensure appropriate hiring practices were followed. Licensing must receive written confirmation that corrections have been made no later than November 12, 2025.

    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): Policies and procedures of the facility have not all been reviewed annually as required. LO Vermeulen and the facility manager looked specifically at the emergency evacuation policy and determined that it required to be updated.

      • Observation(s): The Resident Bill of Rights has not been communicated to persons in care orally prior to this inspection.

    • R1.1 - Does the Licensee operate in accordance with their license and inform Licensing of any significant changes?
      • Observation(s): The Resident Bill of Rights has not been communicated to persons in care orally prior to this inspection.

      • Observation(s): LO Vermeulen reviewed person in care records with facility manager. Immunization status is not currently being recorded. This information is required by Section 78(1)(a) of the RCR.

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Hot water found to be 54 degrees Celsius at time of inspection. Please ensure water accessible to persons in care does not exceed 49 degrees Celsius in order to prevent risk of injury.

    • R1.1 - Does the Licensee operate in accordance with their license and inform Licensing of any significant changes?
      • Observation(s): LO Vermeulen discussed with the facility manager the current method of self-monitoring the physical environment, and the care and services provided by it, to ensure that the requirements of the Act and Residential Care Regulation (RCR) are being met. A system of monitoring for health and safety is currently in place but does not include all areas required by the RCR.

    • R3.1 - Do the manager and employees meet the employment requirements and are they assessed regularly for performance?
      • Observation(s): A few staff have had their first aid/CPR expire recently, staff are signed up for new courses; inform licensing on how residents will have immediate access to an employee with valid first aid/CPR at all times and on what systems will be put in place to ensure staff who require first aid/CPR will have it renewed prior to expiration.

    • R9.1 - Are medications stored, handled, and administered appropriately?
      • Observation(s): Handwritten medication changes/additions to the MAR observed during this inspection, inform Licensing on what systems are in place to ensure medication changes are recorded on the MAR by the facility Pharmacist.
      • R9.1C - Ensure a pharmacist packages all medications and records all medications on the person in care's medication administration record; 69( 1 )(a)(b)
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): Care plans do include a recreation and leisure plan, however facility charting does not include how residents are meeting or attempting to meet the goals identified in their recreation plans. Inform Licensing on what systems and audits are in place in regard to residents meeting the goals identified in their recreation care plans.
