      • Toddler 2 and Preschool room: Push pins are used. Please remove the push pins as these may fall onto the floor and injure a child.

      • Baby room: A plastic bag with lunch box inside was observed near the diapering mat. Please remove the bag. Violation corrected onsite.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. There are multiple electrical cords by the diapering area in the baby room. Children can access these cords during diapering.Please ensure the electrical cords are completely out of reach of children.2. OSC: One of the shelves (play refrigerator) was easily moveable. Please ensure all tall furniture are secured.

      • 1. In the toddler room, one of the electrical outlets was missing a cover. Please ensure electrical outlets are covered with approved covers. 2. OSC: Glass jars are used and accessible to the children. Please ensure glass jars are stored away from the children when unsupervised and only accessible to the children under direct supervision. 3. OSC: The area around the printer was cluttered with electrical wires. Children had access to the electrical cords. Please ensure children do not have access to the electrical cords. Violations were corrected onsite.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Paper towel is not available to dry hands in the laundry room. Please ensure all hand sinks are equipped with hand washing supplies.

      • Preschool room: The antiseptic wipes in the preschool room were expired. Please replace.

      • Preschool room: Porous and uncleanable cardboard was placed on top of a shelf. Please ensure all surfaces in the facility are smooth, non-porous and easily cleanable.

      • 1. OSC: The areas around the hand sink and underneath the sink are water damaged. Please repair.

      • Baby room: Children's bottles were kept right by the diapering surface. Please ensure children's bottles are stored away from the diapering surface.

      • Art & crafts supplies- Primo Bingo dabbers in preschool room were labelled 'not intended for children use'. Corrected onsite and removed.

      • First aid kits- some items missing (cpr barrier device, emergency blanket, cleansing wipes). Will send updated list of items for facility.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Spacing- some mats with sleeping children in Toddler room 2 were too close to each other, less than 2ft apart with head to head orientation. Corrected onsite. Ensure for sleeping mats, mats are provided with 60cm (2 ft) distance between children for head to head orientation.

      • Main entrance- the front door was not secured properly (door slightly ajar from dead bolt extended). Corrected onsite. Ensure door is closed properly after use for pest proofing.
