    • 03. Are clients at this facility being protected from hazards (Critical)?
      • -There were 3 white storage shelves stacked on top of each other in the JM 3 Room. Please ensure that these are secured to the wall so they cannot tip.November 10, 2025 - Shelves were secured using what appeared to be double sided tape. The tape was coming loose and not providing adequate support.

      • -Walls in the washrooms on the second floor and in the hallway had chipped and peeling paint. Please ensure walls are smoothed and finished so that they are easy to clean.November 10, 2025 - walls had been patched but not smoothed so they were easy to clean

      • -Several sleeping mats were worn out the corners and/or have holes. Please repair/replace so that they are smooth, waterproof and easy to cleanNovember 10, 2025 - sleeping mats were temporarily repaired with tape until new ones could be ordered.

      • -Blind cords in the Toddler Room were not secured out of children’s reach. Please secure.

      • -Walls in the washrooms on the second floor and in the hallway had chipped and peeling paint. Please ensure walls are smoothed and finished so that they are easy to clean.

      • -Chlorine solution in the JM 3 Room was at 50ppm and was not being changed daily. Please ensure Chlorine is at 100ppm and is made fresh daily. It is recommended that one person makes the solution for the whole facility for consistency.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • -There was no warm water at handwashing sinks at the facility. Please ensure warm water is available. For handwashing sinks a temperature of 37C is recommended.

      • -There are hazardous areas, rooms, or conditions which are accessible to clients in this social care facility:*Raw eggshells were accessible to children. The operator indicated that they are only handled by staff, while wearing gloves. Please do not leave the eggshells accessible to children.*Blind cords in the Toddler Room were not secured out of children’s reach. Please secure.*There were small items in the Toddler Room that were potential choking hazards. Please ensure all potential choking hazards are removed.

      • - There were several types of disinfectants available but it was unclear which ones were being used on the different surfaces. Please organize disinfectants and ensure that manufacturer’s instructions are followed for each one. Only Chlorine can be used on diapering surfaces.
